General Transcript Information

• Your official transcript is a comprehensive record of your LPN courses completed at J. Everett Light Career Center.

• If your student account is in good standing and clear of any holds, you may obtain a copy of your official transcript. To settle outstanding financial obligations before requesting an official transcript, contact Barb Johnson at 317-259-5269 x44080.

• Your transcript request may be submitted in person, via U.S. mail, or using our online LPN Transcript Request Form.

• We can provide you with your transcript via U.S. mail or you can come to JEL and pick-up your transcript in person at the JEL Bookstore (please contact Barb Johnson to schedule an appointment).

• You can pay by cash, check, money order, or credit card (pick-up only). Payment is non-refundable. *Payable to J. Everett Light Career Center

NOTE: JEL does not provide third-party student verification inquires. Students should directly contact our office for a copy of their transcript.  

 

To request a LPN Transcript (Two Options): ​

1. Pick up

   • Fill out the Online Form, then pay for the transcript(s).

     • We accept cash, money order, check or credit cards.  ​

   • Once payment is received, you will be contacted when it is ready.

   • Note: Please allow 48 hours to process.

2. Receive by mail

   • Print and fill out the Request LPN Transcript form. 

   • Send form and $10 non-refundable fee (check or money order payable to J. Everett Light Career Center) to:
