Prior to the start of school, parents may reach the District RN/Coordinator of Health Services for Washington Township Schools by calling 317-259-5265 ext. 44089 or by email.(mailto:rwest@msdwt.k12.in.us?subject=Health Services Inquiry (via jelcc.com)) The student's home high school should share this information some time during the first few weeks of school. To ensure continuity of care, parents should make contact through one of the two methods above prior to the start of school.
Authorization for Prescribed and Non-Prescription Medication Forms may be found on the MSDWT website.(https://www.msdwt.k12.in.us/health-services/) The completed form and medication are to be placed in a sealed envelope with the number of tablets written on the outside and turned in to Rebecca West or Nicole Cooper at J. Everett Light Career Center. A phone call must be made to Nicole Cooper at 317-259-5265 ext. 44017 alerting her that the student will be dropping off the medication.
The medication may also be dropped off by a parent/guardian. Students are NOT permitted to carry or self-administer medication unless the Authorization to Carry and/or Administer Form is completed by a physician and on file with Nicole Cooper.
The high school credits you earned at J. Everett Light Career Center were reported to your home school and are part of your high school transcript. Please contact your high school for a copy of your transcript which will include your credits earned at J. Everett Light Career Center. J. Everett Light Career Center does not have record of this information.
